Instructions:
Complete the form filling in
the swimmer's name, an email address where you can be
contacted,
events the swimmer will participate in the meet.
If
the swimmer has specific requests, these should be indicated on the form.
Submit a
separate form for each swimmer.
Links to available meet packets are
provided for your convenience.
Click
the submit button to email the form or reset to clear the form. After you
have submitted your entries, please check the
On Line Database in 6 hours to
confirm your entries. Meets
highlighted in GREY are CLOSED for entries or have been completed
